2. Download the PS Form-1583 and fill-up all the necessary information except "3a" (we will assign a mailbox number to you).
3. Sign in front of the Notary Public or hand-over the form in-person and sign in-front of the associate.
4. Provide two(2) forms of identification (valid IDs: Drivers License, Passport, Govt. ID card, Vehicle Reg. Card, Voter's ID)
5. Mail/deliver all the documents and copy of IDs including check payment to:
