STEP 1: Download and complete form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store6053@theupsstore.com), or call us 904-225-8840 to follow up on the application. We'll also need to get 2 forms of identification, and payment.
