HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store5876@theupsstore.com),
or call us (847) 726-9696 to follow up on the application. In order to start receiving mail, we will need to see 2 forms of identification, and a payment method must be arranged at that time.
