Required Forms To Open a Mailbox
1. Valid driver's license/ ID Card.
2. And one of the following with a matching address: If the documents do not match then a 3rd document is needed. That can be from a governmental entity or recognized financial institution or a utility bill with the applicant's name and current permanent address may be used for such purpose. Both Forms must be either brought into the store or mailed with copy of the required IDs.
