       **When opening a mailbox please Bring 2 forms of ID. Valid forms of ID include: Armed Forces, government, university: Passport, alien registration card, certificate of naturalization: current lease, mortgage or Deed of Trust, voter or vehicle registration: or a home or vehicle insurance policy.*

 

 HOW DO I SIGN UP?

 From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store !  Just follow the steps below and you'll be on your way!  It's easy:

 STEP 1:  Download and complete form 1583.  (This is the Post Office's "Application for Delivery of Mail through Agent"). 

 STEP 2:  Complete the required (*) fields below and click "Print" to print our Mailbox Agreement.  

 STEP 3:  Come into the store, or call us at (814) 464-1566 for pricing or current specials and to follow up on the application.  We'll also need to get 2 forms of identification (see list above), and payment.
