HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print PSform 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store5059@theupsstore.com), or call us (214-219-5550) to follow up on the application. We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
Two types of identification are required. One must contain a photograph of the addressee(s). Social Security cards, credit cards, and birth certificates are unacceptable as identification.
Acceptable identification include: State Identification card; armed forces, government, university, or recognized corporate identification card; passport, alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or a home or vehicle insurance policy. A photocopy of your identification may be retained by agent for verification.
