Required to open a Mailbox:
1. Photo ID: Drivers License
2. One of the following:
a. Passport
b. Government issued document such as a vehicle registration card
c. Home or vehicle insurance policy with a matching address
d. A utility bill with a matching address
(Sorry, but social security cards, credit cards, and birth certificates ARE NOT accepted as forms of identification.)
There are just two to forms to complete. The first is the USPS Form #1583. It must be signed in front of us or a notary. The second form is The UPS Store Mailbox Rental Agreement Come in the store to fill them out and we'll get you started!
