How to Open a New Mailbox:
1. Fill out the form below and click Print.
2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.
3. To Open a Mailbox, you will also need to complete a copy of the Post Office- 1583 Form. Click here to download a copy of the Post Office- 1583 Form.
4. A The UPS Store representative will contact you to answer your questions.
5. Bring the Form(s) to the store and Open your New Mailbox. These forms are available online, and in the store for your convenience. .
Two forms of ID are required: One MUST have a current address. One MUST be a photo ID.
Acceptable IDs: Driver's License, Armed Forces ID, Government ID, Passport or alien registration card. If address is different, a recent utility bill or similar mail must also be shown to match address.
IDs NOT accepted: Social Security Cards, Birth Certificates, Credit/Debit/ATM Cards.
