WEB SIGN-UP INSTRUCTIONS:
    1. Fill out the Web Form below. (This will be E-Mailed to the Store)
    2. Additional Information Needed to Complete the Mailbox Process:
        * 2 Forms of ID for All Members on the Box 
            -One Must Contain a Photograph
            -Cannot use Social Security Cards, Birth Certificates, or Credit Cards
        * Size and Length of Term for Mailbox (Call for Pricing)
        * 4-Digit PIN Number (For 24-Hour Access)
        * Number of Keys Needed (2 Free -- Additional $5.00)
    3. Signatures are Needed on all Mailbox Application Forms and Form-1586.
        (This can be completed when you come in)
    4. We will call for payment when everything is setup and supply you with any
        additional information you may need (Box #, Address, etc.)
