STEP 1: Fill out the form below and click Print. A Mailbox Agreement Form will be printed for you to fill out at your convenience.
STEP 2: A The UPS Store representative will contact you to answer any questions.
STEP 3: Come into the store, or call us at (703) 820-3300 for pricing or current specials and to follow up on the application. We'll also need to get 2 forms of identification, and payment.
