HOW DO I SIGN UP?
From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store in downtown Black Mountain.
Just follow the steps below and you'll be on your way. It's easy!
STEP #1: Download and complete Form 1583. — Must be signed in front of
us or by a Notary. Please call if you need help filling it out.
(This is the Post Office's "Application for Delivery of Mail through
an agent").
This form requires 2 types of ID:
#1 Valid driver's license/ ID Card.
#2 One of the following with a matching address:
• Armed forces, government, university identification card.
• Passport, alien reg. card, current lease, mortgage, or deed of trust.
• Voter or vehicle registration card.
• Home or vehicle insurance policy.
Social Sec. Cards, Credit Cards and Birth Certificates are not acceptable forms of ID.
STEP #2: Complete the required (*) fields below and click "Print" to print our
Mailbox Agreement.
STEP #3: Come into the store, email us, or call us (828-664-9333) to follow
up on the application. We'll also need your 2 types of identification,
Form 1583, the Mailbox Agreement (print below), and payment.
