HOW DO I SIGN UP?
 
From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store in Ft. Wayne. Just follow the steps below and you'll be on your way!  It's easy:
 
STEP 1:  Print out US Post Office form 1583 & The UPS Store  Mailbox  Agreement .
 
STEP 2:  Complete the form 1583 and The UPS Store Mailbox Agreement.
 
STEP 4:  Make sure that you have two of the approved forms of identification. The acceptable forms of identification are listed in section 8 of the form 1583.
 
STEP 5:  Bring the completed documents and your identification into the store or Email them to us at   store4682@theupsstore.com or Fax them to us at (260-490-2811).
 
STEP 6: Call us at (260-490-2800), to follow up on your application, get any questions answered and to pay for your Box.
