STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, or email us (store4387@theupsstore.com), or call us (206) 282-2888 to process your application. We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
