STEP 2: Complete the required (*) fields below and click "Print" to print our Mailbox Agreement. STEP 3: Come into the store, email us (store4366@theupsstore.com), or call us (916-817-2233) to follow up on the application. We'll also need to get 2 forms of identification, and payment of course.),
