How to Open a Mailbox at The UPS Store:
1. Fill out the form below and click Print
2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.
3. Bring the form to the store and open your new mailbox.
4. Be sure to bring two forms ID.
