How to Order a New Mailbox:
1. Fill out a Mailbox Agreement and Post Office Form 1583 at your convenience.

2. Bring the Form(s) to the store and Open your New Mailbox.
Two forms of ID will be required-Contact The UPS store for details of acceptable ID's.
Complete, notarized, original forms must be received before a mailbox becomes active.
