HOW DO I SIGN UP?

Just follow the steps below -- It's easy:

 

STEP 1:  Download and print form 1583 http://www.usps.com/forms/_pdf/ps1583.pdf. (This is the Post Office's "Application for Delivery of Mail through Agent").



STEP 2:  Complete the required (*) fields on the *Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.


STEP 3:  Come into The UPS Store, email us (store3977@theupsstore.com) or call us (801)375-1700 to follow up on the application.  We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
