If you are interested in renting a mailbox, please fill in the required information. Also, you will need to bring in two forms of ID from the list below. One of them must be a picture ID .
Valid Driver's License; State ID card; Armed Forces, government, university, or recognized coporate ID card; passport, alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or a home or vehicle insurance policy.
HOW DO I SIGN UP?
From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store#3971. Just follow the steps below and you'll be on your way! It's easy:
STEP 1: Download and complete form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent"). Must be signed in front of us or by a notary public.
STEP 2: Complete the required (*) fields below and click "Print" to print our Mailbox Agreement.
