HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store3860@theupsstore.com),
or call us (262) 697-4310 to follow up on the application. We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
2 Forms of ID Required
Type 1 - Government issued photo ID (non expired drivers license, state ID, passport, military ID)
Type 2 - non expired insurance card, vehicle registration
