HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store3859@theupsstore.com), or call us (419) 429-6245 to follow up on the application. Bring 2 forms of identification to the store, and we will set up a payment method at that time.
Acceptable identification includes: valid driver's license or state non-driver's identification card; armed forces, government, university, or recognized corporate identification card; passport, alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or a home or vehicle insurance policy
