Required Forms To Open a Mailbox
1. Valid driver's license or ID Card.AND
2. One of the following items with a matching address:
* Armed forces, government, university, or a recognized corporate identification card.
* Passport, alien registration card, current lease, mortgage statement, or deed of trust
* Voter or vehicle registration card
* Home or vehicle insurance policy
We will not accept Social Security cards, credit cards, or birth certificates as forms of identification. Both forms of documents must match or a 3rd form of identification will be required.
3. Click here to download form 1583. This form must be signed in front of us or by a notary. Please call or come in to the store for help with filling out the form correctly.

Both forms must be either brought into the store or mailed with copy of the required ID's.
