1. Fill out the form below and click Print.
2. A Mailbox Agreement form will be printed for you to fill out at your convenience.
3. Print a copy of the Mailbox 1583 Form, and fill it out along with the Mailbox Agreement Form.
4. A UPS Store representative will contact you to answer any questions.
5. Bring the Forms to the store and Open your New Mailbox.
Two forms of ID will be required-Contact The UPS store for details of acceptable ID's.
