STEP 1:  Complete the required (*) fields below and click "Print" to print our Mailbox Agreement.  

STEP 2:  Come into the store, email us (store2957@theupsstore.com or call us (336) 545-7997 to follow up on the application.  We'll also need to get 2 forms of identification, and payment at that time)  Thank you.
