How to Order a New Mailbox:
1. Fill out the form below and click Print.
2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.
3. A The UPS Store representative will contact you to answer any questions.
4. Bring the form(s) to the store and open your new mailbox.

Please thoroughly read and sign all forms. As an agent of the US Postal Service, we are required to obtain complete information and provide this information to the Postmaster. Spouses/families may complete one form; however, all others sharing a mailbox must complete separate forms.
It is mandatory that all adult parties applicable to this mailbox present two forms of identification prior to the rental. A social security card, birth certificate or credit card are not acceptable. One form of identification must contain a photograph (see List 1); choose a second form of identification from List 2.
