HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print Form 1583. (This is the Post Office's "Application to Receive Mail Through Agent".)
STEP 2: Complete the required(*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store2778@theupsstore.com), or call us at (323) 721-7529 to follow up on the application. We'll also need to see 2 forms of identification, and we will set up a payment method at that time.
Two types of identification are required. One must contain a photograph of the addressee. Social Security card, credit card, and birth certificate are not acceptable as identification. Acceptable identification includes: valid driver's license or state non-driver identification card; armed forces, government, university or recognized corporate identification card; passport or alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or home or vehicle insurance policy. A photocopy of your identification may be retained by the agent for verification.
