  How to Order a New Mailbox:

  1. Download and fill out a Copy of the Mailbox Agreement Application- Click Here.

  2. To Order a Mailbox, you will also need to complete a copy of the Post Office- 1583 Form.  Click here to download a copy of the Post Office- 1583 Form.

  3. Contact The UPS Store if you have any questions.

  4. Bring the Form(s) to the store and Open your New Mailbox. These forms are available online, and in the store for your convenience.
   
  Two forms of ID will be required-Contact The UPS store for details of acceptable ID's.
