How to Order a New Mailbox:

1. Fill out the form below and click Print.

2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.

3. A The UPS Store representative will contact you to answer any questions.

4. Bring the Form to the store and Open your New Mailbox. This form is available online, and in the store for your convenience.
