How to Order a New Mailbox:
1. Fill out the form below and click Print.
2. A Mailbox Agreement form will be printed for you to fill out at your convenience.
3. You will also need to fill out a copy of the Post Office- 1583 Form, to open a new Mailbox. Click here to download a Post Office- 1583 Form.
4. A UPS Store representative will contact you to answer any questions.
5. Bring the Forms to the store and Open your New Mailbox.
Two forms of ID will be required-Contact acceptable ID's are listed in Paragraph 8 of the Post Office- 1583 Form.
