Required Forms for opening a Mailbox( 2 Forms required)
1) Valid Photo ID ( Driver's License/Passport)
2) Voter or Vehicle registration card, student or employee ID, current utility bill, home or auto insurance card.
3) Completed 1583 Form(click on link to open form). If you are not signing this form at our location, you must have this form, and your photocopied forms of ID notarized.
4) Complete the "Order a new Mailbox" form below to complete the Mailbox Service Agreement.
