In order to set up a mailbox you will need two forms of identification. One needs to have your photo and one needs to have your physical address. Please bring these with you when you come in.
Acceptable forms include: Driver License, Passport, Photo ID, Utility Bill, Leasing Information, Car Registration.

HOW DO I SIGN UP?
Just follow the steps below -- It's easy:
STEP 1: Download and print form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store1055@theupsstore.com),
or call us (805) 492-2279 to follow up on the application. We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
