Small: 3.5" x 5"
Medium: 5.5" x 6"
Large: 10.75" x 6.25"
To place an application for a mailbox:
1. Please complete the following two forms, and submit to the store by fax,
email or in person.
2. Submit two forms of identification: One must be a state issued picture ID such as
a drivers license. The second form can be voter or vehicle registration, health
insurance, US passport, government ID, or utility bill.
3. If the above information is not submitted and signed in person, please have each
of the above documents notarized. The mailbox agreement and the 1583 form
must have a notarized signature. A certified copy is requested for the two forms of
identification.
