    • HOW DO I SIGN UP?


    From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store Greenwich, CT.  Just follow the steps below and you'll be on your way!  It's easy:


    STEP 1:  Print form 1583. 
     

    STEP 2:  Fill out the form after printing, using 15 E Putnam Avenue in Greenwich, CT  06830 as the address to be used for delivery.  Fill out whatever you can, and we'll do the rest when you come to the store.


     

    STEP 3:  Come into the store, email us (store0822@theupsstore.com), or call us (203-622-1114) to follow up on the application.  We'll also need to get 2 forms of identification from the list below, and payment of course.

    Allowable Forms of Identification (Please Bring Two):
    State Driver's License/State ID
    Passport
    Armed Forces Card
    Government, University or Corporate ID
    Certificate of Naturalization
    Current Lease, Mortgage, or Deed of Trust
    Voter or Vehicle Registration
    Home/Vehicle Insurance Policy
