HOW DO I SIGN UP?
From individuals to large businesses, there are lots of ways to take advantage of the mailbox services at The UPS Store! Just follow the steps below and you'll be on your way! It's easy:
STEP 1: Download and complete form 1583. (This is the Post Office's "Application for Delivery of Mail through Agent"). Or, click the link below.
STEP 2: Complete the required (*) fields below and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, or call us at (617) 868-5055 for pricing or current specials and to follow up on the application. Once the application is completed and we receive payment, you’re all set! Please note that you will need 2 forms of ID. If you are unable to come into the store and/or need to setup a mailbox over the phone or via fax / email, we will need notarized copies of both ID’s and form 1583.
