How to Order a New Mailbox:
1. Fill out the form below and click Print.
2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.
3. A The UPS Store representative will contact you to answer any questions.
4. Bring the Form(s) to the store and Open your New Mailbox. These forms are available online, and in the store for your convenience.
Two forms of ID will be required to Open a Mailbox-Contact The UPS store for details of acceptable ID's.
