1.  Fill out the form below and click Print
2.  A Mailbox Agreement form will be printed for your convenience.
3.  A representative from The UPS Store will contact you to answer any questions.
4.  Bring the form to the store and open your new mailbox.

At the time you set up your mailbox, you will be required to provide two forms of ID (per the U.S. Post Office).  At least one of the ID's must have your picture on it and at least one of them must have your current home address.  Acceptable forms of ID are:

Valid Driver's License or State ID Card
ID Card with an ID Number issued by:    U.S. Government
                                                                          State Government
                                                                          University or College
                                                                          Recognized Corporation
Passport or Alien Registration Card
Current Lease
Mortgage or Deed
Voter Registration Card
Utility Bill
Home or Vehicle Insurance Card or Policy
Vehicle Registration
