How to Order a New Mailbox:
1. Fill out the form below and click Print.
2. A Mailbox Agreement form will be printed for you to fill out at your convenience.
3. A UPS Store representative will contact you to answer questions. Please include your phone number.
4. Bring the Form(s) to the store and Open your New Mailbox.
Two forms of ID will be required-Contact The UPS store for details of acceptable ID's.
