How Do I Sign Up?
Just follow the steps below. It's easy.
STEP 1: Download and print Form 1583.
(This is the Post Office's "Application for Delivery of Mail through Agent").
STEP 2: Complete the required (*) fields on the Order a New Mailbox form below
and click "Print" to print our Mailbox Agreement.
STEP 3: Come into the store, email us (store0140@theupsstore.com),
or call us (313) 884-8440 to follow up on the application.
*We'll also need to get 2 forms of identification, and we will set up a payment method at that time.
If you come into our store and complete the transaction for our mailbox service within 24 hours of completing this form, we will give you one month of mailbox service free!
