How to Open a New Mailbox: 

1. Fill out the form below and click Print.

2. A Mailbox Agreement Form will be printed for you to fill out at your convenience.

3. To Order a Mailbox, you will also need to complete a copy of the Post Office- 1583 Form.  Click here to download a copy of the Post Office- 1583 Form.

4. A The UPS Store representative will contact you to answer any questions.

5. Bring the Form(s) to the store and Open your New Mailbox. These forms are available online, and in the store for your convenience.
 
Two forms of ID will be required-Contact The UPS store for details of acceptable ID's.
